The contact form collects the name, email address and short message you choose to provide. It exists to arrange a free 5 to 10 minute consultation, and nothing more.

Please do not include clinical details, diagnoses or crisis information in the form. Email and web forms are not secure channels, and a website message cannot be monitored for emergencies.

Submissions are used to respond to your inquiry and to schedule a consultation. They are not used for advertising and are not sold or shared with third parties for marketing.

Information is retained only as long as needed for that purpose, or as required by law and professional recordkeeping obligations.

Once you become a client, your protected health information is handled under HIPAA and Florida law, and is described in the Notice of Privacy Practices provided at intake. That notice (not this page) governs your clinical record, including the limited circumstances where disclosure is legally required, such as risk of serious harm or suspected abuse of a child or vulnerable adult.

This site may use standard hosting and analytics services that record technical information such as pages viewed and general device type. That information is not used to identify you individually.

Amber is licensed as an LMHC in Florida and New York. Services are provided only to people physically located in Florida or New York at the time of the session.
